Risk Factors for Acute Exacerbation Following Bronchoalveolar Lavage in Patients with Suspected Idiopathic Pulmonary Fibrosis: A Retrospective Cohort Study

چکیده
Introduction: Bronchoalveolar lavage (BAL) is useful for diagnosing diffuse lung disease and excluding other conditions. However, acute exacerbations (AEs) are recognized as important complications of BAL in patients with idiopathic pulmonary fibrosis (IPF). This study aimed to identify risk factors BAL-induced AEs IPF. Material methods: We retrospectively analyzed the data 155 suspected IPF who had undergone between January 2013 December 2018. BAL-related AE was defined development within 30 days after procedure. compared clinical features parameters (AE group) without (non-AE group). also reviewed relevant reported literature. Results: Among patients, 5 (3.2%) developed BAL. The average duration from onset 7.8 (2–16 days). Results univariate analysis revealed PaO2 < 75 mm Hg (p = 0.036), neutrophil content ≥ 7% 0.0061), %DLCO 50% 0.019), Gender-Age-Physiology (GAP) stage III 0.034), recovery rates 30% 0.001) significant post-BAL AE. All five recovered were discharged. Conclusions: Disease severity, high levels BAL, poor may be AEs.

Bronchoalveolar lavage pepsin in acute exacerbation of idiopathic pulmonary fibrosis.
Some patients with idiopathic pulmonary fibrosis experience acute exacerbations in their respiratory status leading to substantial morbidity and mortality. Occult aspiration of gastric contents has been proposed as one possible mechanism leading to these acute exacerbations. We sought to determine whether pepsin, a marker of gastric aspiration, is elevated in bronchoalveolar lavage fluid obtain...
متن کاملBronchoalveolar lavage as a possible cause of acute exacerbation in idiopathic pulmonary fibrosis patients.
In the past 13 years (1980 to 1992), bronchoalveolar lavage (BAL) was performed on 124 idiopathic pulmonary fibrosis (IPF) patients (29 F and 95 M, 60 +/- 1 years, mean +/- S.E.) at Tohoku University Clinic and Hospital. Among them, three patients showed acute exacerbation immediately after BAL and died of progressive respiratory failure after 2.5 weeks, 2 months and 3.5 months, respectively, d...
متن کاملRisk factors for an acute exacerbation of idiopathic pulmonary fibrosis
BACKGROUND Acute exacerbations of idiopathic pulmonary fibrosis are major causes of morbidity and mortality among patients with idiopathic pulmonary fibrosis. However, acute exacerbations remain unpredictable. The aim of this study was to investigate risk factors for acute exacerbations of idiopathic pulmonary fibrosis. METHODS We performed a retrospective cohort study of patients with idiopa...

Cobalt has also been shown to be a skin sensitizer, and lymphocyte sensitization had been demonstrated in patients with cobalt dermatitis by means of patch tests and in vitro lymphocyte stimulation tests.7 This provides a pote...
متن کاملPrognostic Factors in the Acute Exacerbation of Idiopathic Pulmonary Fibrosis: A Retrospective Single-center Study
Objectives Acute exacerbation of idiopathic pulmonary fibrosis (IPF-AE) has been recognized as a fatal pulmonary disorder, but the exact prognostic factors are unknown. The aim of the present study was to analyze the clinical characteristics of patients with IPF-AE and identify the prognostic factors. Methods The medical records of 59 cases of IPF-AE were retrospectively reviewed. Clinical data...
